Provides simple and practical guidance to managers, supervisors and operators on how to recognise and deal with typical human-factor problems involving alarm systems in the chemical and allied industries. It aims to prevent future accidents similar to the one at the 1994 explosion and fires at the Texaco Milford Haven refinery which injured twenty-six people and caused damage of around #48 million and significant production loss. Contents: Who should read this guidance and why? Approach; Are there problems with the existing alarm system? Ask operators and safety representatives about their experiences; Ask managers about alarm issues, eg; How are new alarms added and existing ones modified? Is the alarm system designed to a standard that takes human limitations into account? Decide what to do and take action; Check and manage what you have done; Conclusion; References; Further reading. |
