Abstract Provides simple and practical guidance to managers, supervisors and operators on how to recognise and deal with typical human-factor problems involving alarm systems in the chemical and allied industries. It aims to prevent future accidents similar to the one at the 1994 explosion and fires at the Texaco Milford Haven refinery which injured twenty-six people and caused damage of around #48 million and significant production loss.
